About Me

Carly Boehm, M.S.
Senior Transition Coach
I discovered my passion for helping the geriatric population about 10 years ago when I started working in an amazing assisted living facility during college. I received my Bachelors in Psychology in 2015 while working in the facility. I continued working there after graduation and quickly realized how much the geriatric population was mentally suffering. I also noticed how many questions families have about what step to take first or what to do next to better help their loved one and keep them safe. I thought there must be some way to make this inevitable part of life easier to understand. I continued my education and received my Masters in Psychology. I helped out in the assisted living facility for some time and then decided to go out into the community and help out other individuals and families better understand and cope with the aging process as a senior transition coach.

What does a Senior Transition Coach Do?
I am here to help you with anything you need. It generally falls into three categories: In-Home services, Facility care, and move management. I offer family mediation at any point during the senior transition process. Discussing these matters amongst the whole family is encouraged. Anyone who wants to can participate in sessions. I am here to support you, the family, the aging individual, and to help you all support one another. I can educate you on all non-medical aspects of the aging process. I can answer any questions and help resolve concerns you have revolving your aging loved one from "Are they still safe to be driving?" to "Should they still be living alone?" to "What is hospice care? What does that mean?". I am here to offer support in anything you need as you decide what the aging process looks like for you and your family.
